What you need to know if you should go off on Short Term Disability
Once you have called your team leader and advised him or her that you will be off for more than seven calendar days, your team leader will enter STD Pending in S.A.P. this action will generate the process with Manulife and Access HR and let them know you will be off on Short Term Disability (STD).
Access HR will send you a short term disability kit by mail which needs to be completed.
You will need to fill in the Employee statement and also be required to have your doctor fill out the Attending Physician’s Statement. You must submit these two forms to the Disability Management (DM) provider within 14 Calendar days following the start of your absence. (The DM provider requires both forms in order to render a decision).
We recommend that you indicate your request to be represented by APOC, by doing so you are assured to be well represented by a specialist if required at the Final Appeal Stage.
It is advisable for you to also take a copy of your job description with you to the doctor, so that they know what your job entails. You can access your job description by clicking on the following link on Intrapost.

Once you’ve done this and the page opens up, you need to scroll down to the Job Descriptions then click on the one that applies to you. Print a few copies to take home and bring one to your Doctor so that he/she will know what your job duties are in order to help him/her fill out the paperwork required by the DM provider.
